How to Apply

1. Complete the Application
Fill out all required information in the online Citizens Academy application.

2. Sign and Upload the Consent Form
Download the consent form, review and sign it, then upload the completed document with your application.

3. Submit Your Application
Review your information for accuracy and click Submit. You will receive confirmation once your application has been received.

  • Fill out the application completely.

  • A PDF reader is required to view and fill-out applications.

  • All applications must be signed and notarized before acceptance.

  • Once completed scan all documents and fill out the form below and attach all necessary documents.
